ACL tear with bucket handle meniscus tear: what happens now?
An ACL tear with a bucket handle meniscus tear is treated with surgery: where possible, the torn meniscus is brought back into place and repaired while the ACL is reconstructed. Exercise or injections will not put the displaced meniscus back. Dr Sankalp Pande stresses that time matters most, because the earlier the surgery, the better the chance of saving the meniscus. Saving it keeps the knee better for longer and lowers the risk of early wear and tear.
Key points from the video
- In a bucket handle tear, a long part of the meniscus (the knee's cushion) tears, moves out of place and gets stuck, so the knee may lock or not straighten.
- If your knee is locked, do not force it straight and do not delay; get it examined quickly.
- Exercise or injections will not put the meniscus back; exercise is needed before and after surgery, but surgery is the treatment.
- Time is the most important thing: if you wait for months, the meniscus is often left too damaged to repair.
- The aim is to save as much meniscus as possible, repairing it while the ACL is rebuilt, which makes the repair's chances even better.
- Under SAFE ACL, his ACL treatment-planning approach, surgery is planned by examining you, not just the report, so timing of injury and lifestyle change the plan and recovery.
- Recovery: exercises from day one, knee bend up to 90 degrees for 3 weeks, office and driving at 6 weeks, gym at 6–8 weeks, sport after strength and stamina tests.

Questions patients ask
Can a bucket handle meniscus tear heal with exercise or injections?
No. In most bucket handle tears the piece of meniscus has already moved out of place, and exercise or injections will not put it back. Exercise is needed before and after surgery, but surgery is the treatment. He says injections have no role.
How soon should a bucket handle tear with an ACL tear be operated on?
Do not delay. The earlier the surgery, the better the chance of saving the meniscus by repairing it. If you wait for months, the meniscus is often left too damaged to repair.
Will my knee be damaged after an ACL and bucket handle tear?
Saving the meniscus is meant to protect the knee. When the cushion is preserved, the knee stays better for a long time and the risk of early wear and tear is lower.
When can I walk, drive and go to the gym after surgery?
Exercises start on the first day. For 3–4 weeks you walk carefully with support, without full weight. After 6 weeks you walk fully, return to the office and can drive. Gym at 6–8 weeks, running at 3–4 months, sport only after strength and stamina tests.

Your MRI report said: ACL tear with a bucket handle meniscus tear, the doctor said you will need surgery, and you got scared. What will happen now? Will I be able to walk normally again? Could my knee be damaged forever? Don't panic.
An ACL tear along with a bucket handle tear is common, seen in 11-42% of patients, according to a 2006 American study, and it can be treated completely. After the right treatment, most patients return to their normal life, such as normal walking, going to the office, travelling, and they can do sports and gym too. According to 2025 research from Sweden, in 160 patients, about 85% of cases recovered.
First, let's understand: what is a bucket handle tear of the meniscus? The meniscus is the cushion of the knee, like a car's shock absorber. In a bucket handle tear, a long part of this cushion tears and moves out of its place and gets stuck inside the knee, so the knee can get locked or may not straighten fully. If your knee is locked, do not force it straight and do not delay surgery at all. Get it examined quickly. If your knee locks, remember, makeshift fixes don't work here.
Now, what is the ACL? The ACL is the most important rope in the knee. It stops the knee from slipping forward and from twisting when you turn. If the ACL is torn, it is hard to keep your balance on the knee. It feels like the knee is darting back and forth, and you feel this most while going down stairs and while running.
A completely torn ACL does not heal on its own, so a new ligament is rebuilt in its place using a graft. This is called ACL reconstruction. And in a knee that has lost its balance, a bucket handle tear often happens, so both need to be treated together.
Now, many patients watch YouTube videos and ask, will it get better with exercise or injections in the knees? Straight answer: in most bucket handle tears, the piece of meniscus has already moved out of its place, and exercise or injections in the knees will not put it back. Exercise is needed before and after surgery, but surgery is the treatment. Injections of any kind have no role at all, there is no evidence for them in any research.
Now, something few people know: in a bucket handle tear, time is the most important thing. The earlier the surgery, the better the chance of saving the meniscus by repairing it. If you wait for months, the meniscus is often left too damaged to repair. So do not delay surgery for too long.
SAFE ACL is an up-to-date approach for ACL and bucket handle tears. Here, surgery is planned by examining you, not just by looking at the report. Suppose two patients have the same report.
The first: a 22-year-old kabaddi player. The injury is 1 week old, and he has to get back on the mat. Raids, turns and jerks are a normal part of his sport. But the injury is fresh, so the chances of repairing the meniscus are very good.
The second: a 42-year-old teacher, with an injury that is months old, whose job involves standing all day. She keeps teaching, and keeps going up and down stairs. For her, we first have to see how much meniscus is left. Same report, but different timing of injury, different lifestyle, so the surgery is different too, and the recovery is completely different.
But one thing is the same for both: trying to save as much of the meniscus as possible, because this is the best treatment for everyone. Where possible, bringing the torn meniscus back to its place and repairing it, and reconstructing the ACL during the ACL surgery. If a car's handle breaks, you don't throw away the car, you repair the handle. Natural is always the best. In the last 10 years or so, knee surgeons across the world and a lot of research papers from America and Europe have come to the same view: repairing is important, rather than cutting away. And when the ACL is rebuilt at the same time, the chances of repairing the meniscus are even better.
Now your biggest fear: what if the knee gets damaged? This is the real purpose of saving the meniscus: when the cushion is preserved, the knee stays better for a long time and the risk of early wear and tear is lower. In research from Germany, about 9 years after repair, the wear in 81% of knees had not increased. This treatment is not just for today, it is for the next 50 years of your knees.
And now, let's talk straight about recovery. For recovery, we use the international standards and latest guidelines that the best ACL and meniscus surgeons or hospitals in America and Europe follow. Recovery of both the ACL and the meniscus goes on side by side, but the pace is set by how the meniscus is healing. Knee exercises are started from the first day. Getting the knee fully straight is the most important thing. For 3 weeks, the knee should bend only up to 90 degrees.
For 3-4 weeks, do not put full weight on the leg, walk carefully with support, after that, for 2 weeks, do exercises to bend the knee fully, then after 6 weeks, you walk fully, return to the office, and you can start stairs and daily activities. You can drive at 6 weeks and join the gym at 6-8 weeks. At 3 months, jogging and brisk walking are allowed, at 3-4 months, running and the start of normal life, and return to sport only when you pass the muscle strength and stamina tests.
Yes, it is very important to say that, because of special implants, the cost is a little higher, but this little extra time and cost is for the safety of your knee. If you don't have the surgery now, you will have to get the knee replaced later. This is not me; a 2025 American study that included 11,500 people says that the chances of knee replacement go up 2.2 times. So what happens now? As per SAFE ACL: the right planning, surgery and complete rehab.
If your MRI says bucket handle tear along with the ACL tear, send your report on WhatsApp, we will look and tell you whether your meniscus can be repaired or not. The number is on the screen and in the description too.
